OK, the patient came in for a wound repair.  what makes the office visit significantly separate??  What did the provider do that was significantly separate from the repair to the wound that makes the e/m medically necessary?  What medical condition supports the office visit?  Modifier 25 states the E/M should be significant and separate from the procedure.
